Do proinflammatory mediators play a role in early recurrence after lumbar discectomy? Correlation of interleukin 1 polymorphisms and the recurrence risk

Objective: Lumbar disc disease (LDD) is a complex condition in which environmental factors and multiple genes are expected to exert influence. It is reported that besides the described mutations of collagen genes, polymorphisms within proinflammatory mediators like the Interleukin-genes are associated with intervertebral disc degeneration.
Methods: Biopsy samples from 288 patients with symptomatic LDD, who had undergone lumbar discectomy were analyzed. Half of the study group (case) underwent re-discectomy between the first and third postoperative year due to early recurrence. The other half (control) stayed relapse free with a minimum interval of 10 years. Genotyping was performed in order look for single nucleotide polymorphism (SNP) in the genes of IL1A/IL1B, IL1RN.
Results: We found a significantly higher incidence of the following SNP in the case group compared to the control group. In the IL1A/ IL1B gene: rs6716046 with 76% in the case group and 70% in the control, p=0.0376. In the IL1RN gene: rs11677140 with 79%/71%, p=0.0292 and rs315936 with 36%/33%, p=0.048.
Conclusions: Our study suggests that interleukin 1 polymorphisms could be associated with a higher early recurrence rate after lumbar discectomy.
