The Cochlear Implant Centre Ruhr: a potential model of integrative supply

Introduction: Since 1995 deaf or severely hearing impaired patients are provided with cochlear implants at the CIC Ruhr in Essen. The treatment concept would potentially fulfil the requirements of SGB V §§140a-d concerning the integrative supply. The principles of this law edited in the year 2000 basing on the participation of different professional groups and the maxim „out-patient before in-patient“ are consequently realised in Essen for more than 10 years.
Methods: Until 2005 93 children, 12 adolescents and 32 adults, were implanted (median age 5 years). Speech perception in adolescents and adults was tested with the Freiburg speech test for numbers, the modified Aachen Aphasia test and speech tracking. In children semi-quantitative tests (Schmidt-Giovanni, Pollack) as well as the Mainz Speech Test I in closed set and the Göttingen Children’s Speech Perception Test I in closed set were used.
Results: Speech perception in the Freiburg number test showed an improvement from 23% preoperatively to 72% in adolescent and adults after the first 6 months with CI. In the modified Aachen Aphasia test without labiolexia percepted words rose from 45,4 to 124,8 six months after CI implantation. In children the mean value in the Mainz Speech Test I in closed set was 87% after 36 months. Six months after implantation the test score in the Göttingen Children’s Speech Perception Test I in closed set had increased six fold and in the Schmid-Giovannini test it had doubled.
Conclusions: The concept of rehabilitation on an outpatient basis after CI implantation shows comparable results in speech development and hearing perception compared to in-patient rehabilitation.
