Comparison of silicone oil and C3F8 tamponade in retinal detachment associated with closed globe injuries managed with pars plana vitrectomy

Background: To evaluate the effect of tamponading agents in long term structural and functional outcome of Retinal detachment (RD) in closed globe injuries.
Methods: A retrospective chart review of closed globe injuries in one year. All cases underwent routine ophthalmic examination. B Scan was done in presence of vitreous hemorrhage or suspision of occult scleral tear. Patients with RD underwent surgery either scleral buckling or vitreous surgery. Type of surgery was dependent on presence of vitreous haemorrhage, extent of the holes, proliferate vitreoretinopathy and duration of retinal detachment. Anatomical success was defined as attachment of the posterior pole for 6 months following surgery. Functional outcome was defined as change in visual acuity.
Results: Out of 17 closed globe injuries, 9 eyes had retinal detachment. Of these, scleral buckling and vitrectomy with C3F8 tamponade was done in 4 cases, scleral buckling with vitrectomy with silicone oil tamponade was done in 1 case,vitrectomy alone with C3F8 tamponade in 2 cases and scleral bucklingin 2 cases.4 of 9 eyes (44.4%) developed redetachment. C3F8 was used as an internal tamponade in these eyes. After second surgery with silicone oil injection, retina was attached till the end of follow up. Hence, Anatomical success was achieved in 5 eyes (55.6%) after primary surgery and 100% after second surgery.
Conclusion: Silicone oil tamponade achieves good long term structural and functional outcome compared to C3F8 for retinaldetachment in closed globe injuries.
