Purpose: To demonstrate the possibility and efficacy of comprehensive reconstruction long after trauma in an eye that has become phthisical due to extensive anterior and posterior segment scarring. The presenting visual acuity was bare light perception.
Material and Methods: Combined anterior and posterior segment reconstruction.
Results: Surgery included the following steps: iris diaphragm /pupil reconstruction; ciliary body debulking, including removal of the previously retained posterior capsule; cyclodialysis suturing; vitrectomy with removal of subretinal proliferations; endolaser cerclage; and silicone oil implantation.
Conclusions: Modern vitreoretinal surgery allows reconstruction of eyes that may have seen “lost’ only a few years ago. In this eye, the phthisis process has stopped and has not recurred even after silicone oil removal, and the visual acuity reached 20/30 two years postoperatively.
