Long-term results of conventional detachment surgery for retinal dialysis

Background: Retinal dialysis is a frequent cause for retinal detachment in infants and young adults. We report our long-term results with conventional detachment surgery in these special cases.
Methods: 55 eyes of 50 patients with retinal dialysis recieved conventional detachment surgery between 1.1990 and 12.1998. In 2007 45 eyes of 41 patients from these group (40 eyes with buckle, 5 eyes with cryo/laser retinopexy) were examined for long term results (follow-up 9-17 years, median 12,4 years).
Results: Preferred locations of the dialysis were inferotemporal (72%) and superonasal (10%). Macula was detached in 84%. One year after surgery retina was completely reattached by one surgery in 94%, by two surgeries in 98%. Visual improvement was noticed in 70% of the eyes. Long-term follow-up revealed no case with redetachment of the retina, but in 5/45 eyes additional laser retinopexy for lattice degeneration/atrophic holes was performed. Macular pucker surgery and consecutively cataract extraction were performed in 4 eyes. In 5 eyes the buckle had to be removed because of dislocation, pain or double vision. In general all eyes with retinal dialysis revealed more vitreous floaters than the not affected eye contralaterally, but 95% of the patients got accustomed to this condition.
Conclusions: Long-term results of conventional surgery for retinal dialysis are exceptionally good.Preserving of the lens and ability of accomodation are essential advantages of this surgical approach esspecially in these young patients. Financial interests of the author are disclosed.
